How to Apply

Check Eligibility
Our programs serve individuals with diagnosed mental health conditions who meet income requirements. Priority is given to those experiencing homelessness or housing instability.

Fill out our comprehensive application form online or in person. Our staff can assist you if needed.

Provide proof of income, mental health diagnosis, identification, and any other required documentation.
